INSTRUCTIONS: Before you begin completing this survey, please read the following instructions.
The survey has 8 questions 
Some questions will simply require you to choose an answer out of options provided by marking the box. Others will ask you to fill in the space provided by typing. Try to select the most appropriate answer to each question. In some instances you may not know the answer, so please tick the 'Don't Know' option. Some questions will appear automatically if you answered 'Yes' to the question before it. Please answer all questions presented to you on the page.

A percentage bar at the top of each page will tell you how far you have progressed through the questionnaire. If you wish to save your answers and resume the questionnaire later, please click on the Resume later>> button. You will be asked to enter an email address, username and password. An email will be sent to your nominated address with a link back to the page you finished on. You may also return to your survey via the original link using your username and password.

In order to progress through the questionnaire, please remember to use the following navigation buttons.
Do not use browser arrows (Forward/Back) to navigate as data will not be saved.  
Click the Next>> button  to start the survey and move forward. Please maximise screen to ensure full questionnaire view.

Click the Previous>> button to return to the previous page 

Click the Resume later>> button if you would like to save your responses to date and return later. 

Click the Load unfinished survey>> to load a previously saved survey 

Click the Submit>> button to submit  and save your entire survey
Please be aware that if you click Exit and Clear Survey >> then all of your answers will be deleted and you will have to begin a new survey.

All data will be stored securely in a password protected database in a secure building in a hospital facility.
